Community Health Workers

What is a Community Health Worker?

Community health workers are trusted members of their community who help people connect to and access health and social services. They are neighbors, friends, relatives, and staples of their community. They are also advocates, educators, counselors, and supporters of those who need help navigating the health care system.

One word: Superhero.

What is a Community Health Worker?

Community health workers are a core part of a health care team dedicated to achieving optimal health. They have significant impacts on community health and individual health outcomes, as well as decreasing health care costs, improving patient satisfaction, and improving treatment. They are available to assist you in getting the health care you need through a variety of services, including, but not limited to:

  • Health education and information
  • Service coordination and navigation
  • Coaching and social support
  • Advocacy
  • Individual and community capacity
  • Direct service
  • Individual and community assessments
  • Outreach
  • Evaluation and research
  • Translation and interpretation (Marshallese and Spanish)
